Upon entering the office you will be greeted by our friendly staff. At the counter, you will be asked for your picture ID and insurance card. Once copied, the staff will hand you a clipboard with the necessary paperwork to fill out in order to properly establish you as a patient in our office. (By the way, if you click on the "New Patient Forms" item of our website you can download, print, and fill out the forms prior to your first visit, saving you time in our office.) Once all the necessary paperwork is filled out and processed, and your insurance benefits have been verified and discussed with you, our staff will bring you to an examination room. We also accept self-insurance patients.
In the examination room, you will first have a consultation with Dr. Puentes to discuss the details of your visit. Depending upon the results of your examination and your conversation with the doctor other in-office tests, such as the ultrasound, may be performed. Other more laboratory extensive tests may also be scheduled. Each patient is different and the circumstances of their needs will be determined to suit their best medical interests.
